The Living Well Partnership brings together four well-established practices in Southampton, with seven sites, which have joined forces to create a super-partnership that can deliver exceptional clinical care. Our mission is simple: to enhance the health and extend the lives of our 40,000 patients and you can be an integral part of our clinical team in the role of Advanced Nurse Practitioner.

Main duties of the job

We are looking for enthusiastic ANPs who want to embrace the changes in general practice but value the preservation of continuity. Joining our multi-disciplinary Urgent Care team, you will be at the heart of providing excellent care while general practice is in a state of evolution.

We are 13 partners with many salaried GPs, ANPs, pharmacists, practice nurses, HCAs, reception and administrative staff. We care for approximately 40,000 patients in a variety of demographic areas from inner city to rural. We are a training practice and we also undertake undergraduate medical training in several sites and are looking to expand our training portfolio.

Overview of your organisation

Southampton has a dynamic health economy and range of commissioned services that support our work including evening and weekend hub appointments via our Federation, self-referral to physiotherapy and MSK pathway, and Urgent Response teams to support admission avoidance.

Your work locations may vary, dependent on the needs of our patients. All of our sites are within a 5 mile radius of one another. We welcome part-time and full-time applications.
