Our mission is simple: to enhance the health and extend the lives of our 45,000 patients and you can be an integral part of our clinical team in the role of Advanced Nurse Practitioner.

We’re looking for an experienced nurse who, acting within their professional boundaries, will provide care for our patients, including initial history taking, clinical assessment, diagnosis, treatment and prescribing and evaluation of care. The candidate will demonstrate safe, clinical decision-making and expert care, displaying excellent assessment and diagnostic skills, for patients within the general practice.

In this role you’ll share responsibility for telephone triaging patients who wish to be seen on the day by a health care professional, diagnosing and treating patients presenting with minor illness and advising people in respect of their continuing medical and nursing needs. You’ll prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner.

Our clinical team is made up of 21 Non-Principal GPs, Advanced Nurse Practitioners, Practice Nurses, Paramedics, Physician’s Associates, Health Care Assistants, Social Prescribers, Care Coordinators, and our own in-house Pharmacy team.

We are a part of a community that has a dynamic health economy and range of commissioned services that support our work, including evening and weekend hub appointments via our Federation, self-referral to physiotherapy and MSK pathway, and Urgent Response teams to support admission avoidance.

Your work locations may vary, dependent on the needs of our patients, but all our sites are within a 5-mile radius of one another.
