Who can we help?
- Patients registered with one of the Living Well Partnership practices.
- Patients aged 16 and over.
- Patients experiencing one or more long term conditions or at risk of developing one.
- Patients in need of support with identified health and wellbeing goals.
- Patients wanting to develop skills and strategies to manage their own health and wellbeing.
Our Health and Wellbeing courses and workshops are designed to help you live well.
They are delivered face to face, by telephone or online in approximately 8 x 45 minute sessions.
