We can complete all sorts of reports for you, from life insurance applications to HGV licensing medicals.

Some reports require a physical examination, others are completed via a review of your medical record. Your documentation will make clear what’s needed.

LWP use an external company called Medi2Data to complete most types of factual reports for us.  They are able to complete them significantly quicker than we can internally. We have been through our normal vetting process to confirm they work to the same high data protection standards that we do. More information on their service is available below.

To send us a report request please submit an eConsult outlining what you need and our Records Team will contact you with advise on next steps and to ensure we have the correct paperwork. To save time in processing your report please include the following in your request;

• Do you agree to your report being completed by Medi2Data if appropriate (you will also be agreeing to us sharing your email and phone number for them to use to contact you in regard to your specific request only)?
• Would you like to review the completed report prior to the report being sent to a third party, or are you comfortable with it being sent direct to the third party (if applicable to your report)?

Please note that the NHS considers medical reports as private work and as such there is a charge incurred by the patient in most cases. You will be advised of the charge before the work is carried out. We also get a number of requests for reports or paperwork that do not require GP input. This sort of paperwork can be very time consuming and take GPs away from the front line so we may sometimes decline to do a report/letter and signpost you to another route that can better meet your needs.

What is eMR/Medi2Data?

MediData is a NHS Digital accredited company who have developed a digital system called eMR, which enables Medi2Data to create digital, GDPR compliant medical reports. A registered GP will still sign off your report.

eMR helps GP Surgeries with data security, speed and efficiency

eMR also helps you to easily see your medical data, stay in control of it and decide who you want to share it with. Medi2Data has worked hard to develop their NHS GP IT Futures accredited technology, eMR, which interfaces with our GP practice’s system to extract your medical record. You can receive a full copy of that information securely and share it with others as you wish, keeping your data safe.
