Please do not submit requests for children/dependants under your name, these will be rejected and you will need to re-submit. You will need to create dependant profiles for dependants by clicking ‘Settings’ when you log in to Anima. Click here to watch a video guide on how to add dependents to your profile.
Please select ‘Living Well Partnership’ as your surgery when asked to, not your local site. This won’t affect which site you visit.
