Administrative Queries

Please submit your request for a new or repeat sick note via eConsult.

All sick notes need to be reviewed by a clinician and an appointment may be required.

Your sick note will be emailed to the address on your records or provided by you in your eConsult.

Sick notes are usually issued between 3 – 5 working days, but are usually completed quicker than this.

Please note our eConsult call back service is not available for sick note requests, paper forms are available at each of our surgeries for patients to complete. Paper fit note requests will take longer to review.
